Serum testosterone values (testosteronemia) are expressed in nanograms per deciliter (ng / dL) or in nanomoles per liter (nmol / L); this last unit of measurement is the one adopted by the SI and as such represents the reference standard at the international level.

Calculation of free testosterone
The following form determines the share of free testosterone and bioavailable testosterone, making the calculation on the basis of the serum concentration of total testosterone and its transport proteins. Being a water insoluble hormone, in fact, testosterone must necessarily be associated with specific proteins, such as SHBG - sex hormone-binding globulin - or non-specific, such as albumin. The bond with this last molecule (which represents the most abundant protein in plasma) - as opposed to that with SHBG - is however rather labile.
The amount of free testosterone represents the active hormonal fraction, because it is separated from the transport proteins and ready to enter the cells where it carries out its action; the share of testosterone linked to SHBG instead represents a sort of reserve, temporarily inactive; finally, the fraction associated with albumin, by virtue of the weakness of the bond that unites them, is considered bioavailable (like the free one). In addition to the two classic values (total share and free share), it is therefore possible to calculate a further fraction , that of bioavailable testosterone, adding to the free fraction that bound to albumin.
