Hydroxy beta methyl butyrate- molecule derived from the transamination process of one of the three branched chain amino acids. From leucine, in fact, a ketoacid is generated by transamination, the afa ketoisocaproate, which represents the direct precursor of hydroxy methyl butyrate. A balanced diet, in men weighing 70 kg, can provide about 200/400 mg per day of HMB, a much lower quantity than that commonly used in sports supplementation (3gr), to which ergogenic effects are recorded. The pharmacokinetic and pharmacodynamic properties of this molecule are not yet fully characterized, but the studies present in the literature show a potential capacity to reduce protein degradation and increase synthesis; and this is the much sought after effect in sports integration. The supposed biological mechanisms underlying this effect are yet to be demonstrated, but the experimental evidence intends to exhibit a direct involvement of mTOR (factor involved in protein synthesis) and an effect inhibitory on protein degradation via ubiquitin proteasome The result of this modulation would therefore be a marked improvement in protein turnover.
Other studies, however, underline the ability of HMB to reduce the activation of caspases, with consequent reduction of apoptotic processes, and to protect the cell from the oxidative insult mediated by ROS. This molecule is thus finding ample space in the treatment of all. those pathologies that induce cachexia (loss of lean mass), such as AIDS and cancer.

In sports, on the other hand, the HMB is used both with the aim of improving body composition and with that of enhancing athletic performance (strength and endurance).

As with most commercial supplements, there is an open debate on the ergo-genic effects of hydroxy methyl butyrate. Numerous studies show that supplementation with HMB, for about 4 weeks at 3 grams per day, can increase strength and reduce the feeling of fatigue in training, with a significant decrease in the markers of muscle damage induced by physical activity. Others, however , support the hypothesis of the absence of ergogenic properties of this molecule.
However, from the careful re-reading of the various articles in the literature, it is possible to draw some conclusions, albeit partial, given the "scientific ignorance of the mechanism of action:

• Greater effectiveness in untrained individuals, for whom in addition to the above improvements, there was also an increase in lean mass.

The elective dosage appears to be 3 grams per day.
